#Biguanides – this medicine originated from a plant. Goat’s rue, used for hundreds of years for blood sugar regulation, was analyzed and found to contain guanides.

This medicine can reduce the liver’s production of glucose, reduces intestinal absorption of glucose, and enhances cells’ uptake and metabolism of glucose. Metformin can allow weight loss in patients and does not cause weight gain, water retention, or hypoglycemia.

Drug names: Glucophage; Glucophage XR (brand name regular); Glumetza, Fortamet (brand name extended release) metformin HCL (generic regular); metformin ER (generic extended release)

Side effects: Gastrointestinal distress can occur in up to 33 percent of people with diabetes, including nausea, flatulence, and diarrhea. It is best to titrate this medication up slowly as a result.

Note: If you are experiencing gastrointestinal distress on typical metformin, ask your physician to switch you to metformin ER, which the vast majority of people with diabetes can handle, even those who cannot handle straight metformin.
